Insurance Plans
To facilitate insurance billing, patients need to complete an insurance form for use by the Center's billing service. You may complete this form when you are seen for follow-up or sick visits, receive allergy shots or need to obtain new vials of allergy extract. Our new business office is located in Carrollton and staff will be available in the office or by phone to answer your questions. The Center's business office can be reached at:

Plan Participation
We participate in most insurance and managed care programs. Most of these plans require members to pay an office co-payment for each office visit. Payment of the co-payment will be expected at the time of your visit. We will file a claim to the plan for the balance.
These contracts are frequently reviewed and changed; therefore, we will ask for a copy of your ID card at each visit. Prior to your visit, please contact us or your health plan to verify our participation in your plan.
If there is a problem with verification of your insurance information, full payment for services rendered will be expected at your visit. Therefore, it is incumbent upon you to ensure that you provide the practice with your most recent and correct insurance information.
